Reporting a Concern

These report forms are designed to allow any University community member to fill it out and provide as much detail as possible.

Each form addresses different areas. Please feel free to reach out if you have any questions regarding the report forms. Don’t worry if the form you filled out was meant for a different department we will route it to the correct one.

Reporting Forms

Academic integrity violations are behaviors that include, but are not limited to, cheating, plagiarism, fabrication, facilitating academic dishonesty, misconduct in research and creative endeavors, misuse of intellectual property, or multiple submission.

This form is for reporting all accidents, injuries, or near-miss incidents occurring on Florida Polytechnic University property or during university-sponsored activities. Your prompt and accurate reporting is crucial for ensuring the safety and well-being of our community, complying with safety regulations, and improving our campus environment.

Any accessible barrier, including physical or digital barriers may be reported. The ADA Coordinator works to address these concerns.

Students, employees, and community members are encouraged to use the online referral form below to refer a student to CARE Services and report a concern for a student’s well-being.

Any applicant, employee, or student who believes they have been discriminated against or harassed based on a protected class. Further information on Non-Discrimination.

If you have witnessed or have knowledge of a potential hazing incident, please report it immediately. 

Non-academic violations are behaviors that include, but are not limited to, alcohol, disorderly conduct, drugs, hazing, harassment, physical violence, or other violations of university policies, regulations, or rules.

Sexual misconduct includes, sexual assault, sexual harassment, intimate partner violence (dating violence and domestic violence), or stalking behaviors. The Title IX Office works to address these concerns.

Students can express thoughtful concerns where no other forum exists and to provide a final appeals pathway for processes that either do not have an appeals pathway or where existing pathways have been exhausted. More information on the Student Complaint and Appeals Process.

Session Details

This session will demonstrate that practical risk management is for everyone, regardless of a formal program. Attendees will learn actionable and simple strategies that are easy to implement, enabling them to start immediately by focusing on their top risks to build greater operational resilience and ensure the sustained success of their auxiliary enterprises.

Presenter Profile

Michelle Powell serves as the pioneering Risk Manager at Florida Polytechnic University, the state’s sole 100% STEM-dedicated institution. Having been with the university for nearly 11 years, Michelle transitioned from a leadership role in Admissions in October 2023 to establish and evolve the risk management function from the ground up. In this solo capacity, Michelle builds robust frameworks for our dynamic, young university, overseeing our insurance portfolio, consulting on third-party and event risks, and developing critical campus-wide training programs. Michelle has obtained the Committee of Sponsoring Organizations (COSO) Enterprise Risk Management certificate and the Associate in Risk Management (ARM) and Construction Risk and Insurance Specialist (CRIS) designations. Her distinct background in mathematics and engineering, combined with extensive higher education leadership, brings an analytical and strategic approach enhancing the institution’s resilience.
